Grievance form

This is a form for you to lodge any grievance that you have relating to the Australian Society for Parasitology Inc. (ASP) and their associated events. This form can be used to lodge your grievance anonymously. Please note that if you wish the ASP to respond to this grievance then you will need to either leave your contact details on this form or contact one of the ASP member contacts listed below.
1.Please name the ASP event or activity that relates to your grievance.
2.Please write your grievance below in detail.
3.Please write the date, time and location (s) of the activity that you are lodging a grievance about.
4.If you do not wish to remain anonymous please write your name and contact details below. The ASP Secretary will contact you using this information.
